Premarin Vaginal Cream Information

Premarin Vaginal Cream is a locally acting estrogen replacement therapy (ERT) used to treat vaginal and urinary symptoms of menopause, including atrophic vaginitis, vulvar thinning, and painful sexual intercourse. It works by delivering conjugated estrogens, a mixture of different estrogen hormones, directly into the vaginal tissues. This replaces the estrogen naturally lost during menopause, working locally to increase tissue thickness and moisture in the vaginal mucosa.

Side Effects

Commonly reported side effects include vaginal itching, irritation, or an increase in clear or white vaginal discharge. You may also experience breast tenderness or pain, headaches, back pain, abdominal or stomach cramps, nausea, diarrhea, and a higher vulnerability to vaginal yeast infections.

Because vaginal estrogen is absorbed systemically into the bloodstream, it carries the standard risks associated with estrogen therapy:

• Cardiovascular Disorders: An increased risk of developing blood clots (such as deep vein thrombosis or pulmonary embolism), stroke, and heart attack.

• Dementia: Possible increased risk of developing probable dementia, particularly in postmenopausal women aged 65 years or older.

• Gallbladder Disease: Postmenopausal women have an elevated risk of developing gallstones or gallbladder inflammation.

• Malignant Neoplasms: Increased long-term risk of breast cancer, ovarian cancer, and endometrial (uterine) cancer if used without a progestin in women with an intact uterus.

• Vision Changes: Potential for structural vision changes or decreased vision clarity.

Seek medical attention immediately if you experience sudden pain, swelling, or redness in one leg, severe chest pain, coughing up blood, or shortness of breath, a sudden, severe headache, confusion, or difficulty speaking, or a new lump in the breast. Consult your healthcare provider before stopping this medication, as your menopausal vaginal symptoms may return.

Precautions:

Do not use Premarin Vaginal Cream if you have a known hypersensitivity or anaphylactic reaction to conjugated estrogens or any ingredient in the cream formulation. Tell your doctor about your medical history, especially if you have:
o History of or active deep vein thrombosis (DVT) or pulmonary embolism.
o History of, active, or suspected breast cancer or other estrogen-dependent tumors.
o Known or suspected pregnancy.
o Known thrombophilic disorders (such as Protein C, Protein S, or antithrombin deficiency)
o Recent heart attack, stroke, or severe liver disease/dysfunction.
o Undiagnosed or unexplained abnormal vaginal bleeding.

Premarin Vaginal Cream may interact with liver enzyme inducers such as carbamazepine, phenobarbital, rifampin, and St. John's Wort, which can speed up hormone metabolism and reduce the cream's local effectiveness. Because it is absorbed systemically through the vagina, it can also alter requirements for thyroid replacement therapy or certain blood thinners.

Premarin Vaginal Cream is strictly contraindicated during pregnancy and may cause severe fetal harm if used. Estrogens are present in human milk and can reduce milk production in breastfeeding women; therefore, use during lactation is generally not recommended. It is recommended to speak to your doctor immediately if you are planning to get pregnant.

Premarin Vaginal Cream can weaken and damage latex condoms, diaphragms, and cervical caps, rendering them ineffective. For women with an intact uterus, this medication should be prescribed with an appropriate dose of a progestin to prevent endometrial hyperplasia.

Store Premarin Vaginal Cream at room temperature (20°C to 25°C) in a dry place. Keep the tube tightly closed when not in use, and do not freeze. Keep the cream and its applicator safely out of reach of children and pets.
